I don't know, maybe this goes without saying but I'll say it anyway.
Sometimes patience is the best medicine.  Lots of skin contact, all feedings
should take place cheek to breast (I know this is difficult when giving a
bottle but do the best she can).  Lots of warm baths together.  I've seen
this take weeks.  This baby needs to be wooed to the breast.  Create lots of
positive experiences there forgetting about the breastfeeding component for
awhile and focusing on the mothering, comfort component only. Stop poking
things into baby's mouth unless he readily accepts them, become very tuned
in to his body language and respectful of what he is trying to express.
What I see happen when a mother patiently works through it this way with a
reluctant baby is that one day baby "just latches" and then things progress.
I think too often it takes more time and patience than many mom have or feel
they can give to the process.
