Trophic feeds on day 1-3 of life are not new - they are the standard of care.
Most neonatologists do not want to start feeds this early because of the
association of early FORMULA feeds with necrotizing enterocolitis. The only
safe feeds (usually o.5 to 1 cc every 3 to 6 hrs in a baby < 1500 gms) are
mothers' own breastmilk and pasteurized donor human milk. The average amount
of colostrum I see in these Moms is 1- 10 cc per pumping - so we can keep up
until the full milk suplly arrives. Diluting colostrum wilh sterile water is
OK, but will not have the same positive effect on GI motility.
