Hello everyone,

My daughter had her first baby 3 weeks ago. They had a rough labor and delivery but breast feeding by 10 hours of life. He has a significant upper lip tie and a visible/palpable posterior tongue tie. After 4-5 days, sore nipples healed and the baby stopped the occasional "chewing". Around Day 6, baby started having around 15 ml of emesis a few times a day which is becoming more frequent. He is gaining well, and urine/stool output is good. Had his second check up and all was well.

My daughter is concerned because he fusses at the breast from 9 pm to 6 am: takes the breast initially, but arches and pulls off after a few minutes, and refuses to latch back on. Then cries because he wants to be fed. The rest of the day, the little guy is happy and content on and off the breast.

I suggested pumping/hand expressing through the first let down prior to feeding and laid-back breastfeeding thinking that she may have a strong initial let down. I also encouraged her to see her local IBCLC for another perspective as we live 2 1/2 hours apart.

Her local IBCLC thinks that her volume is low overnight. Her suggestion was to pump in the morning and either bottle or syringe feed the pumped milk prior to the night feedings to calm the baby, then offer the breast. 

Any advice, suggestions, wisdom will be greatly appreciated!
