Welcome to Lactnet.
I have had many clients (mothers) have severe side effects to reglan:
parkinson-like tremors, cognitive issues, nystagmus, severe diarrhea.
There have been no infant side effects in my practice.  I no longer even
mention that drug to mothers.  I do refer to a breastfeeding medicine
specialist physician if a mom needs a galactogogue or treatment for
polycystic ovary syndrome or hypothyroidism, or evaluation for hormonal
imbalances.  She often prescribes domperidone, which is now mainly
available by mail from overseas pharmacies.  Domperidone has been very
effective for my clients, and I have had only one who had a headache on
the full recommended dose, and was able to tolerate a reduced dose.  No
other side effects in mothers or babies.
I do mention fenugreek when the milk supply deficit is mild, or the
mother does not have the resources or desire to see a doctor or take a
prescription medication.  Some moms are convinced that it worked, but as
we also corrected the baby's latch, increased feeding frequency, and
added pumping at the same time, who knows if the fenugreek was even
necessary.  There was one preliminary (uncontrolled pilot) study done on
fenugreek, and it seemed to have positive results.  I forget the authors
(very well regarded physician LCs).
