First time I ever saw this was in a woman who came to our clinic for routine follow-up after early discharge, and her main concern was low supply on day 4 or five. She was breastfeeding pretty much continuously and baby was not ever showing signs of satiety, nor had baby stopped losing weight.  In the course of the appointment it came out that she was incredibly thirsty, and while in my office (as we waited for the internist to come and examine her) she drank no less than 3 litres of water and was thirsting for more.  She was treated for the diabetes insipidus which was unknown before that appointment, and she never developed much of a milk supply.  This was her second baby. First baby had done all right though she was never one of those women with milk for triplets.  Next baby she struggled from day one, and never managed to build up much of a supply either.  And with her NEXT baby she decided to opt for formula from the start, since her experiences from the previous two children gave her little reason to think it might work the next time.
I've only seen one other case, also one with sudden onset in the early postpartum period, and this one resolved within some weeks.  Breastfeeding went fine.  Probably has to do with the gravity of the insult to the pituitary - one was transient, the other, permanent.  Both were fascinating to work with and both were picked up due to things noticed in connection with the start of breastfeeding.
