I had a  mother recently like that. I could actually feel what felt=20
like "pitting  edema" on and around her areolas. She was extremely engorge=

and I  was unable to get anything out with a pump.


One other visual: If I see or am told upon asking, that mom's ankles are  
still swollen even after the birth, especially after the first few days, I often  
also see really challenging breast engorgement, and difficult or impossible  
latching ( and baby has usually already been either underfed due to poor or no 
 latch and no/low output of milk, then overfed on fast bottles of formula, 
when  the mom "gives in" and tries the bottle.) Today, mom said even her hands 
were  still swollen. A few handfuls of parsley, cold compresses, RPS, etc as 
mentioned  with the mom knowing that when the ankles and hands seem more normal, 
and if  she's kept up with suggestions, things should begin to turn around.
