I had this with both kiddos, and it was just that the milk ejection 
reflex was a hard squeeze.  Tell her it's a good sign in that she knows 
she's letting down.  BTW, for me it hurt much less when my supply 
ramped down at 6 and 12 weeks, and eventually disappeared.  It was 
directly related to how full the breast was.  More frequent feedings 
and just knowing what it meant made it much more tolerable.
