Hi Marie, this is a cronic problem in too many hospitals, mine included. If it was during the night and I meet the mother the next day, I don't say much about it because it is water under the bridge, the horse is out of the barn. An argument can be made that adequate sleep helps decrease postpartum depression. Do not flame me yet. I have found it counter productive to do any kind of scolding, blaming, or shaming with patients or staff. I like to work with the mothers whereever they are at. "Keep your baby in the room with you as much as possible and offer the breast with all of baby's oral fdg cues so you can both practice and learn how to nurse!" I have voiced the following information at the Unit Council Practice Committee that babies are more awake during the night and that is a great time for them to practice breastfeeding, that the mothers who do that go home more confident and with a greater milk supply. Mothers need to nap during the day.
The following 5 practices increase successful breastfeeding rates:
1 breastfeed within the 1st hour 2. Only breastmilk is fed to infants 3. rooming in
4. No pacifier use 5. Telephone # to call if problems arise is given to all mothers at discharge.
If your hospital is not doing all of these things, you may want to work on one or two at a time. Be friendly, find allies.
