I would like some input about this situation:  I haven't seen this mom.  She
lives about 2 hours away and we have talked on the phone.  An LC  has visited
her once and helped with positioning, waking techniques, and said the baby
doesn't curl his tongue around her finger at first but then after a few tries,
begins to curl and suck normally.  The mother delivered her 7th  child July 9.
 Her oldest child is 10 years of age.  She has breastfed all, with varied
lengths of time.  The last baby she nursed for 2 years.  She received an
epidural about 45 minutes before the birth. It was a wet tap and they used a
blood patch.   The blood patch had to be repeated yesterday (day 5 postpartum)
due to severe headache.  She has taken anaprox only.   Baby boy weighed 7 lbs.
13 1/2 oz.  Mom had blood pressure problems during pregnancy and took Aldomet.
 She has used Aldomet during lactation, however, her blood pressure is staying
at 130/80 without the meds now.  She has not experienced breast fullness,
leakage, and baby latches, sucks a little and then falls asleep, she thought
she was doing breast compressions but really has only been massaging her
breast towards the nipple while he feeds.  He does swallow a couple of time
while she does this, but not alot.  He is wetting 3 disposable diapers a day,
but no bowel movements since she left the hospital on Sunday.  The newborn was
difficult to wake and slept for long periods until day 4 and was latched on
for 22 hours! She did give about 1 oz. of formula with an eye dropper, then
later 1 oz. in a bottle.   She says he appears to be starving, but when he
does latch on he goes to sleep.  Her milk looks more like mature milk when she
expresses (small amounts).  Mom has not had mag sulfate, nor does she feel
that she has a retained placenta fragment ... no fever, passed only one clot,
more like a normal period than postpartum discharge.  She is very thirsty,
even after receiving IV fluids when they did the blood patch yesterday.  She
is drinking .. drinking.  She takes the baby to the ped. today because of the
no bm's.  The hospital staff told her to give formula with Karo syrup last
night.  She didn't.

My suggestions so far are ...  proper breast compression, stimulating the
opposite nipple while breastfeeding, using the position and latch that Dr.
Jack teaches, if must supplement don't use a bottle. She has an SNS, but has
not used it.   What else?  This mother is a breastfeeding counselor with WIC,
soft spoken, and when asked how things are going she says fine, but has shared
all of this with me.  Father works nights and sleeps until around noon,
returns to work at 6:00 p.m to midnight.  She assures me that she isn't doing
too much.
