Hi Kathleen,
I know you are concerned about all the infants and looking at providing
evidence to change practice, but if your immediate need is to effect the
change in care that your future grandchild gets, you might try this:
Have mother write a simple birth plan. Provide refs as far as STS
(regulates glucose, maintains temp and homeostasis), early bf, any
protocols you can find re hypoglycemia (Acad of BF Med and AAP), see if the
OB and Peds will agree and sign it. If all else fails, mother can refuse to
have the glucose testing done if baby is well and falls out of the high
risk categories, and is showing no s/s.

I am pretty sure that the AAP has a policy because our rural hospital,
which is not the most progressive but our nsy director is pretty up to
date, has a hypoglycemia policy which does not test well babies but does
test infant of diabetic mother, LGA, SGA, preterm. He usually does whatever
the AAP says to do. I believe the early acceptable numbers are above 25,
and as the infant gets hours older the last one should be above 45.

I've been cancelled quite a bit and did not work at all last week, and not
sure about this week, but I can try to email you the particulars when I'm
at work. My work email is [log in to unmask]
