After chasing up amazon here in the uK, I received this message:

Anyone know if this applies to the USA as well?  Has the book been
withdrawn, or is this a question of never having been supplied to the UK???
I ordered it fairly reluctantly becasue I thought I was going to have
violent disagreement with it, and its expensive, but I wanted to see what it
was like.  I am intrigued by knowing that it is unavailable...
