Hi friends,

I have two questions...One is a quickie.  Does anyone know the chemical in
cabbage responsible for its antiinflammatory affect?

I had a patient who delivered a 31 wkr., small but stable in the nicu.  Mom
b/f 2 others without problems.  She has lupus.  She became preeclamptic, ended
up with a c-sec.  She began pumping religiously at day 2, q2h day and eve.,
once at night, despite her unresolved high blood pressure and intermittent
elevated temp.  She developed at clot beneath the c section incision and was
taken back to surgery.  Two days later she dehisc'd.  Through all this she was
getting nothing when pumping and beginning to obsess about it.  She was in bad
shape.  The doc felt her obsessing was really steroidal psychosis.  The
urologist got her blood pressure under control.  The steroids were monitored
better and she was d/c on 3 antihypertensives, 3 weeks postpartum. Prior to
leaving she did some skin to skin and got a drop.  Near d/c the perinatologist
suggested she stop pumping for the time. At that point I too felt like her
pumping had been for naught and was contributing to her fatigue. At three
weeks out I thought it now unlikely she could establish a supply.  The baby
was tranferred to another hospital and I lost track of her.   She called the
other day to say that after being home 9 days she felt she had regained her
strength, had a more balanced attitude about breastfeeding and returned to
pumping and skin to skin.  The baby is now 41 weeks, home for 2 weeks and b/f
3/4 of the time with the intent of getting to full b/f.   I was pleasantly
surprised.   My question... Given her physical state for the first 3 weeks was
i exhausting her by supporting her pumping regime.  What are the chances that
her milk would have come in anyway, once she was no longer in a catabolic
state.   Or did the initial pumping keep that door open?  I expect to run into
this type situation being in a high risk center.  Understanding the events
would help me guide other moms.
