That's amazing that they want moms to  pump that early.  My comment is that 
if you have a mom that has  labored for at least a period of time and her baby 
goes to the NICU, and she  pumps within the first hour to two after birth, you 
will get a nice bolus of  colostrum.  If you wait for 4 or 5 hours, you will 
get drops (if you are  lucky).  If it is a planned C/Section, and she hasn't 
labored, I suspect  all you will get is drops around the flange.  In that case, 
hand expression  is definitely appropriate for the first little bit, and then 
continue pumping  frequently.  The oxytocin of labor and pushing will help 
push the colostrum  where it is available.  It isn't the thickness per say, it 
is the  location.....
