Valerie,
  I think you are asking important questions and pointing up the
reason it is so essential that bf advocates be at the forefront
of this conversation, not coming up the rear. We don't like to
acknowledge that bf can sometimes be harmful, and we are so
frightened that formula will take over that we don't deal with
these issues.
The opposite should be true. We are the experts and we have an
obligation to  be the ones formulating and asking the hard questions
and making sure that those are the questions before researchers,
funders and policy makers. If we don't Ross Labs and others will.
This also holds true in the issue of HIV and breastfeeding.
