Kathy has described what has come to be a very common issue with the ACA
and breast pumps. While her patient had a breast reduction and may not be
making much milk, she at the very least deserves the best pump possible for
her situation. If she was issued a pump that is not sufficient for the
situation she can appeal this to her insurance company. If that does not
work, she can report this situation to her state's insurance commissioner
and ask for some sort of recourse. She (or Kathy) should send her insurer a
copy of the Model Policy for Payer Coverage of Breastfeeding Support and
Counseling Services, Pumps and Supplies at

If all of this fails to help, she could go to the media with her plight to
see if unfavorable publicity would help budge the insurer into doing what
is right.
