Hi Winnie,
I personally don't use many small shields, I was just responding to a
situation where they were already using a small shield.

If mom's nipple and some of her areola don't fit into the shield, I
don't use it.  I like to have Medela small and newborn shields, and
Ameda shields (which are wider) on hand so I can choose the correct fit
for the individual mother.

Barbara Wilson Clay researched palate length, and found that the small
shields were all that would fit into the mouths of some of the babies
she measured.  She mainly uses small shields.

And yes, we all like to turn them part way inside out to "pop" as much
of the breast as possible into them.
