Hello, wonderful ladies!

I have a client, 25, primip. Low milk supply. On low dose of domp (4 pills,
once a day as prescribed by her Registered Midwife). Some improvement seen.
She comes from a long line of breastfeeding women (YAY!!).

4 years ago, she had a benign brain tumor in her cerebellum. She had it
successfully removed. she had steroids for a while after surgery (she can't
remember name of steroid). She is no longer on it, for a couple of years.
After surgery she had mobility issues (needed to learn to walk all over
again). Her left side still is not as strong as her right. She admits to
'babying' the left hand side because of that.

Interestingly, her left breast, although the side with pain, has responded
wonderfully to the domperidone. I don't see much drinking on her right hand
side, on which the baby has always latched better..

She was taught breast compression and hand expression by me to increase
milk transfer and milk supply. Unknown how compliant she is when I'm not
there.

I'm wondering if the brain surgery has anything to do with the low milk
supply, or if it's just one of those things?  She didn't have optimal
latching prior to my involvement (I saw her at 9 days postpartum). I think
that may be the largest impact. That and the midwife told her it's ok to
let a poor gaining baby sleep for 4 straight hours at night because mom has
to sleep. Baby nursed twice in 8 hours :(

As an aside - and in full venting mode - why do people think pumping will
tell mom how much milk she is making? This midwife is the THIRD person in
as many days who has told a client that. It's really starting to tick me
off. These poor moms think that's the only way to help them, their baby,
and the health care provider. My counselling skills have been put to great
use this past week... moms need a hug, good advice, and no guilt.
