I agree that in MOST cases, cold for breast engorgement will usually work
more rapidly than heat.

However, there is much to be said for what makes the mother feel better.
Sometimes a bit of warmth in advance of cold will relax her.  It is
important to explain why we are doing what we are doing or suggesting. This
will help people averse to cold to reconsider it when heat is not what we
think might help.

We need to beward of ALWAYS and NEVER statements.  Inevitably, we will run
into someone or some situation that is the exception to the general rule.
Becoming ritualistic about ALWAYS offering something or NEVER allowing
something else is less helpful than reiterating that mothers we assist are
the ultimate decision-makers.
