Sara,
Can you phone another IBCLC in your area to mentor you?  Perhaps there's
someone in your area who's been an IBCLC longer with whom you can collaborate on
this particular case.  Suck-training sounds relevant here, but usually despite
a tongue that's not extended far past the gum line, using an exaggerated,
asymmetrical latch will result in a sustained compression of the breast that's
adequate for milk transfer.  We know that the milk supply has been sustained, so
that's a good thing.  Wouldn't it be helpful if you could partner with a
seasoned LC in your area to address the needs of this baby?  Or am I just
dreaming.....
