This detail got me thinking:

So the baby is not getting the milk....poor milk transfer seems to be
the issue here and not the thyroid.  
What often happens in subsequent births is that the nipple tissue
becomes more elastic and elongated and mother who has breastfed so many
kids is not so careful about latching the baby onto the breast and not
the nipple.  I often see this with mothers who have lots of
breastfeeding experience...they tend to latch the new baby like you
would latch an 8 month old, and this just does not allow the baby to
remove enough milk.  If she has a long nipple, the baby sits on the end
of the nipple and sucks only on nipple tissue, getting just enough to
stay alive, but not enough to grow. 
I have had many moms come to me for help with failing to thrive babies
because of large long nipples, which , by the way, everyone said were
wonderful for breastfeeding!!
