What an amazing and challenging case. They are lucky to have you working
for them.

If mom is breastfeeding, and baby is regulating its intake via paced
bottle-feeding, what then is the issue with weight gain? Especially when
"baby is fine with the gain."

It is the fast growth of the formula-fed infant that creates a problem with
overweight/obese in the 2nd and 3rd decades of life. (Stettler 2009).

This mom is assuring her baby's health. The doctor and nutritionist need
education.

Unless I am missing something??? I am sure the wonderful Lactnet community
will make any necessary corrections.
