It sounds to me like possibly she has some anatomical variation whereby the
milk does not exit the nipples. I would continue the STS and supply line
supplementation, and use reverse pressure softening to see what she can
express. I actually think she will have more success, if any, with hand
expression, but she can try a rental pump. I would recommend one of the top
rentals like Medela Classic or Ameda Lact-E or SMB (my opinion). I would try
something like rescue remedy which might help with letdown (check archives
for this info).
She can continue to breastfeed via supply line for as long as mother/baby
desires.  I would explore which one she is using and try to find one for
long term use she would be comfortable with.
If engorgement again becomes severe and the milk is not exiting, I would use
cold and cabbage to help with involution.
