I have been reading on LactNet for some time now and am a newly certified
LC.  Here is what I am hoping that some of you more experienced LC's have
had experience with or can offer a solution to, mom gave me permission to
post:

Mom, 23, had a breast augmentation at age 20.  The implants were inserted
via an incision at her belly button and placed under her muscle.  She went
from a B cup to a D cup (and is now even bigger).  Since the augmentation,
she has had no sensation to pain, cold or hot on her nipples.  She
delivered full term, vag delivery, easy delivery.  Began breastfeeding
after delivery and has had immediate intense pain with latch and during
entire feeding.  Both myself and the other IBCLC at the hospital has seen
her and assessed a very nice latch.  Mom leaks with colostrum at the
anticipation of latch on.  Frequent audible swallows are heard through the
feeding.  We tried mom on a Lactina pump with the 30mm flanges after the
24mm were too painful.  Mom states the 30mm feel better, but she still has
intense pain.

As I observed this mom breastfeed today, she is almost unable to speak a
complete sentence due to the pain as her baby sucks in bursts, pauses, and
then continues to nurse.  Baby is 2 days old today.  She trembles with
pain despite attempts at imagery, deep breathing and shoulder massage
during the feed.  When asked what the pain feels like, she states it feels
like someone has a metal clamp and is twisting her nipple.  Again, baby is
latched well and at detachment, nipple is round and erect, no compression
stripe visible.  No nipple damage is observed.

I am thinking the problem here is nerve damage.  I have read that this
type of augmentation does the least amount of damage to the nervous tissue
of the breast.  Has anyone observed this type of pain before in a mom
who's had augmentation?  Is there any type of pain medication I could
recommend that would block this pain?  Mom has an extremely high desire to
nurse her baby long term but doesn't think she could bear this level of
pain each time she nurses.
