Shingles? Herpetic lesion of some other kind? Or some kind of intraductal
papilloma protruding out of duct opening in nipple?
Better to go to an M.D. and get it checked out with an ultrasound and/or
a mammogram to R/O pathology and seek appropriate Tx.

In the meantime, she could try for some pain relief with either an
icepack, or try a folded washcloth saturated with the hottest water she
can stand on the inner wrist without burning her, and apply it to
nippleareolar complex till it cools.
