The mom who had twins at age 47 after infertility treatments: I would say 
she is definitely at risk for low milk production because I consider this 
type of history as a high risk situation. In this case, I would maximize all 
the BF practicies I know to be helpful for good milk production:
ad lib feedings and skin to skin          rooming-in, household help for mom 
if possible
if babies are poor feeders (35 wks) then add regular milk expression by a 
piston electric double pump
herbs or meds as galactagogues if needed
screen for anything else that might lower the supply like birth control 
meds, cold meds, anemia, thyroid etc.
I can't remember the original post, did mom's milk "come in" around day 3-5?

Re feeling 'antsy' or 'crawling out of her skin' - what meds is she on? I 
had a reaction like this once to Tylenol PM or Benadryl (brand names) once. 
Could she have an anxiety disorder? Surely she is under stress with newborn 
twins. Are they in NICU care or well baby? Maybe she is feeling overwhelmed 
and having a sort of panic attack.

Hives: this has been discussed and will be in our archives. As I recall, 
antihistamines often work like diphenhydramine (benadryl), ranitidine, and 
some of the newer non-sedating antihistamines on the market now.
