The most likely etiology of her fever is 


inadequately treated UTI and possible pyelonephritis.


She needs to be re-evaluated and have her antibiotic 


changed.  My suggestion would be levaquin or floxin. 


Its also possible she may need IV antibiotics with that high of a fever. 


The fact that it increases with breastfeeding is 


likely coincidental.  She can take some 


ibuprofen to control fever AFTER she gets re-checked.
