I already started thinking we are in the outback in the Netherlands... 
hahaha!
Clearly, there are large cultural/regional differences in how the medical 
system works.
Liz, I scrolled through the document you sent a link for, but euhm... I'd 
have to take a week off to fully understand it! hahaha
I focussed on the standards themselve up til now (and not on the 
explanations), but can't seem to find anything about having to ask 
permission beforehand.
Either I have not yet read carefully enough (very well possible) or you were 
trying to make us find that out by ourselves (also very well possible 
considering your inquiring mind!).
I'll have another look at it later on; first back to my daily translation 
quota, now that I work not only as an independent IBCLC but also as an 
independent (book translations) publisher. ;o)

They certainly don't work only under "orders" here (and what
paternalistic and authoritarian overtones that term carries!).

Here in Australia I have been to physiotherapists, optometrists,
dentists, and podiatrists independently of any medical practitioner, no
referral, no "reporting back", and with insurance reimbursement; these
encounters included, at one point, inpatient surgery with no medical
practitioner involved.

Pharmacists, audiologists, OTs, osteopaths, massage therapists, and
speech therapists are all similarly independent practitioners. They may
take advice and referrals from medical practitioners at times, but
consults are also initiatied by clients, and any medical suggestions
(including prescriptions) can be rejected by the independent
practitioner where they feel they are not in the best interests of the
client. They are not "orders".
