Ann,

What came to mind is a breast sling (as seen in Chloe Fisher's book and film)
to gently raise and perhaps with experimenting expose the areola and push the
fatty tissue back a bit; if the sling doesn't support well enough add a
narrow pillow under her breast for support.  Perhaps a sling for baby made
from a good quantity of material might help her if her arms can't reach well.
 Sounds like initially until some aids can be worked out she'll need someone
to help latch baby.  Would she be able to use breast compression to help baby
get more?  Sounds like a real challenge and may take lots of creativity and
trial and error to find a satisfactory position for both.  Best of luck!
