Denise, I'm too am confused about when to use hot or cold on breasts...so I'd 
love to hear other's input.

In the mean time, I just use my common sense:  I know that heat is used for 
getting the milk flowing (as in a plugged duct) and cold is used for swelling.  
So, I always advise heat when the mother has an obvious plug, and if she has 
mastitis, I advise *alternating* heat and cold...heat to keep the milk flowing 
(often just before a feed is a good time) and cold to counter-act the 
inflammation (such as between feeds).

Anyone else?
