I am most intrigued to know what the concern of these parents is.  As far as
I know there is no special threat to babies here...?

Of course, London is pretty crowded, so if this mum comes on her own, I
would advise avoiding public transport where her totally engorged breasts
might be in danger of getting brushed by a member of the public and causing
her pain.

Sounds about right, according to stats I have heard.  Ireland has pretty
dire breastfeeding reputation.  Someone I know (now in her mid to late
fifties) can remember sitting on the stoop of an evening as a child, and
mums nursing their babies out in the street.  This was a two up, two down
sort of house, with 14 kids in the family, and they had to carry the coal
through to cook, and share beds.   There's a sort of synergy about
gentrification / economic 'development' and breastfeeding patterns --
slightly changing in different countries and times, but quite influential.
