First thing that comes to mind for me is areolar edema.  I'm finding a
lot of cases of "flat nipples" are due to this.  I would try Jean
Cotterman's Reverse Pressure Softening.  Some cold on the areolar tissue
might help, but try the RPS first.  I usually hear edematous areolae
referred to as "leathery"
