To possible touch on some of the questions around both use of donor milk and 
description of sterilizing bottles or jars in a cheap but effective way. 
Last December I had the privilege of going to South Africa and observing not 
only how the pasteurized donor milk but also milk of the HIV mother or 
potential HIV mother to give their own milk back to their VLBW premis. They 
used the "Pretoria Pasteurization method" which was so cost effect and cheap 
I was astounded. I do have a PDF format. The article called it " A Potential 
Method for the reduction of  postnatal transmission from mother to child of 
the immunodeficiency virus". Arterial in the Journal of Tropical Diseases 
2000. I don't know if there is a  link at this time. I have seen one the LCs 
from South Africa online here. maybe she has more info on this?

However, the basics is that they use autoclaved peanut butter jars, well 
cleaned of course, and the mothers hand express their milk into these jars 
and firmly turn lid onto the jar. They than procede to boil water in a 
kettle and with a small aluminum sauce pan just big enough so that the water 
can be poured over jar until lid is covered. Once it has sat in this water 
for around 30 minutes or so and has cooled enough they place it in the 
fridge for the use of their premature infant.They are than cup fed generally 
I believe. They can also continue to do this at home in the poorist 
conditions needing less water, small amount of wood for fire or on their 
cooking stove and the jar is much easier to clean than ny other bottles etc. 
It seems to work there can not this method not be adapted around here in our 
western culture or are we to advanced techiquely?

This can also speak to how we ask mothers to sterilize their bottles. Do 
they need to use the tallest bottles. Can they be more creative in what they 
use etc.

I think I asked more questions than I possibly answered. I do believe there 
has been more research made around this method of pasteurization and its 
effectiveness. Probably someone else knows more.
