Worked with a mom today that fits the perfect example of problems with using
PNS to initiate lactation.  Delivered 3#9oz baby IUGR at 36 weeks.  Kept in
hospital 1 week, went home at 3-12.  Mom initiated lactation with classic in
hospital, bought PNS on advice of rental person.  Given nipple shield in
hospital because baby was "nipple confused."  Baby has short frenulum, bubble
palate, now 6#.  Mom's supply down to <30 cc per pump.  Has been feeding
120cc per feed with bottle.  Baby's suck extremely disorganized with raised
back of tongue.  With removing shield and switch nursing and breast
compression baby gained 40 gms. Tried tube at breast, baby refused extra
formula. Switched mom to classic, obtained another 30 cc of milk.  Used
Haberman to get baby to feed, took 5 cc and fell asleep content.  Mom's
nipples were very bruised from PNS plus improper use of pump.  Will follow
closely over the next few weeks.  Thank God there are pediatricians who honor
moms' desire to breastfeed and will refer when they hear a mom complain about
decreasing milk!  This mom has been pumping and dealing with this for >5
weeks.  Hope we are not too late and her supply can be rescued.   Thanks to
the info I learned here on Lactnet, I was able to be alert to possible
problems with the PNS.  I have seen this happen with other moms also, but I
don't think I would have put it together without hearing from all of you
guys.  Thanks to the Kathys for keeping this going!
