AN inquiry into the death of a newborn which died after it was injected at the Labasa Hospital has revealed that the nurse who gave the injection was at fault.

However, according to the director curative Dr Ami Chandra there were factors which contributed to the baby's death.

Rabi islander Kaua Tome's four-day-old son died after he was allegedly give three injections, two in the thigh and one in the hand while it was being breastfed.

A post-mortem examination report said the 3.15kg baby died after he choked on his milk.

Dr Chandra said the report highlighted several factors that contributed to the baby's death including nurse error in injecting a baby being breastfed.
