I have posted about this before.  I made crib cards pink and blue where the 
mom checks the box she feels appropriate.  No pacifiers, breast only, bottle, 
etc you get the point.  At the bottom of the crib card after i list the hunger 
cues, I state, I am learning to breastfeed, and pacifiers can slow down my 
progress.  This is sometimes more for the staff than the parents. ""( We do 
have pacifiers on the unit.  If a box is checked NO pacifiers and staff are 
giving the baby one, this is treated like someone doing a procedure on the 
baby without permission, and they get written up.  Most moms don't even 
have them in the cribs.  When we do a painful procedure like circ's then we 
use them with sugar water for comfort.  Or starting an IV.. Occassionally for 
babies under phototherapy that just won't settle no matter what we do.  But 
they are not routinely given out.  When I worked L&D many years ago at a 
hospital like the one you describe, they were always in the bassinetts.  Maybe 
we have started to come a long way.  One can hope...
