I searched the archives and found one reference to a similar instance in 1996 with no replies.  "unilateral" is not an entry in the index of any of our lactation text.

Mom is 8 months postpartum.  Infant started in the NICU for a couple of weeks with mom establishing her supply via pumping.  She is well versed in hands-on techniques, correct flange fit, etc. When baby began nursing mom reports everything was WNL.  Mom has been an over producer since her NICU days and has donated to our local milk bank.  In generally her left side has been the higher producer, but not with a significant difference.  Last week she started to note that her left supply was dipping which culminated in only getting an 1oz/ pumping session yesterday (typically gets around 5 oz).  Her right side is unaffected.  Baby nurses on both sides and mom has been even more vigilant about putting him to both breasts when they are together.  He does not seem to resist being on the left side.  She has no recent history of plugged ducts or mastitis (had a few plugs as she transitioned back to work several months ago).  Reports mild thrush symptoms and is taking augmentin for a sinus infection - has already increased her intake of probiotics. No known trauma. She works at a local children's hospital and is primarily pumping with a symphony.  She also notes that her right side still feels full as it is approaching time to pump, but her left side no longer feels full at all.  She may possibly have a minor plug on her right side under her armpit and is caring for that accordingly.

Any thoughts on the origin and/or a way to turn it around?  Mom had to dip into her freezer stash for daycare today.  Thank you!
