Do any of you who work in a hospital situation have forms that moms fill out
on release to determine if they were satisfied with the help they received
with breastfeeding in the hospital?
If so, could you e-mail me privately?  We are trying to create such a form
in order to see where we have to improve breastfeeding service, and instead
of reinventing the wheel, I would like to see already existing forms.
