Tandem nursing:  Certainly there are others with more experience, but since
I'm here and have my BAB out...as a Peer Counsellor you probably have the
revised BREASTFEEDING ANSWER BOOK (see pages 347-352) for an extensive look
at this topic.  Common wisdom would say that weaning him right before the
birth would make him more jealous.  Many moms have successfully bf right
through pregnancy and then tandem nursed both children.  It's very
important to discuss how she feels about this and whether she is being
pressured to wean by outside sources.  Some moms' nipples become tender
when they're pregnant, so that may be a factor.  If she's worried about
sore nipples after the birth, please inform her that yes this is possible.
Newborns nurse differently than toddlers.  You might suggest that she try
some USP Medical Grad Modified Lanolin (L_ns_n_h is one) as a preventive
measure (yes it does help prevent soreness, not just speed healing).  She
is going to need lots of household help in the beginning to make sure she
can meet the needs of baby, toddler, herself, her other 4 children, etc.
