Dear Lactnetters,

Another LC and I saw a mother recently who is experiencing severe pain in
breastfeeding her fourth baby.  She is experiencing vasospasms, which we
attributed  partly to the baby's shallow, pinching latch, and partly to
Raynaud's.  This post-feed pain extends medially towards her sternum.  She
also had milk blebs, two on one nipple, and one on another.   We worked with
deepening baby's latch, and suggested she try B6 (not to exceed rec. bf
dosages), and lecithin for the blebs (as well as having them lanced,
although as a physician, she could safely do it herself).  The baby has also
had two CST sessions, and we used gentle TMJ massage to relax his jaw.  We
also discussed the medications for Raynaud's, and I have faxed her the
information about it from Clinical Therapy.

Nursing went much better for about a week, then I heard back from her that:

"Anyway, I had a whole other complication.  I believe the raynauds is also
affecting or causing the rib lesion and thoracic pain I have been
experiencing. I went to a chiropractor - Thursday.  The rib pain was severe
when she adjusted me though the spine felt better.

The complication was severe and I mean severe muscle spasms on that rib
lesion yesterday when I showed up for an adjustment at my local
chiropractor.  I nearly went to the ER but after 3-4 hours was able to stop
the spasm and reduce it to a dull ache."

I wrote her back and suggested that she obtain her letdown first (so baby
wouldn't be clenching and tugging), and encouraged her to contact her
physician wrt Nifedipine (which we had discussed previously).

Anyone encounter another mother presenting this: shooting, severe spasms
radiating towards the sternum postfeed?  Ideas or suggestions welcome.  This
very dedicated mother lives in a small community several hours away, so
doesn't have a lot of local resources, and had travelled here for the CST
and LC visit.  She is about to wean, because of the intensity of the pain.
