This is always a judgement call. Some babies just seem tired, maybe a little
early, 37 wks? etc. Leaving baby STS, but how long do we wait/?
In your scenario, since baby was having resp distress, I vote for immediate
expression of colostrum.
If baby can be placed STS, and has no risk factors like SGA or Prematurity,
then I still think it's a good idea to feed the baby colostrum by spoon
after a couple hours of not eating.
The way I look at it is this:
Will it cause harm to the mom or baby to express and give by spoon? NO
Will it cause harm to the mom or baby to express and have ready if needed?
NO
Will it cause harm to express the first colostrum, even if baby then feels
better and takes the next colostrum (his first feed) at breast? I don't
think so, give him the expressed colostrum after his breastfeed.
Will it cause harm to mother or baby if we don't express? YES it could cause
milk supply problems, baby might have hypoglycemia (if sick) and be fed
formula etc.
So I err on the side of expressing asap.
