I so agree with Jeanette here. One particular case immediately springs
to mind: A mother brings me her 3 month old baby, who is totally
miserable at breast. She had seen a very experienced LC for undersupply
in the first week of his life. Now she is spraying milk from 8 nipple
pores simultaneously and the baby is drowning. Did this LC misassess
here, and mistake oversupply for undersupply? Nope. This very
motivated mom worked so hard on getting her milk supply up that she
overdid it. A few days of managing for oversupply solved the problem.
I had to carefully explain to mom how this situation occured, to
preserve the reputation of my colleague.

We have to give each other a break, unless there is a pattern of the
same issue coming up frequently, then there certainly might be a problem
with insufficient skills, or a knowlege deficit.
