Sorry I'm a little late to this thread. The most likely cause, in my
experience, is pulling and stretching of the nipple. The baby may not be
close enough to mother, the mother may have "chased" the baby  with her
breast, rather than positioning the baby to align with the natural position
of her breast - after latch and when mother lets go of her breast, the
breast pulls away.
Larger breasted mothers often have to support the breast throughout the
feed, esp in the early days/weeks. I often see mothers offering the breast,
but upon latch the mother immediately lets go of her breast.

I show mom how to look for any pulling or stretching or "see-sawing" of the
breast. I tell mom the baby should not only have a mouthful of breast, but
a face full of breast.
