Havining had excrutiating MER's myself - I can offer a couple of
suggestions:

Warm packs, Analgesia (ibuprofin worked pretty well for me - 30 min before
BF gave the most help - time to get into the system), BF frequently - when I
(and other moms I have worked with) had very full breasts, the MER seemed to
be much more painful.  When the breasts were drained frequently, the pain
was less.  Not sure if it was due to not as much milk needing to be pushed
out or what...lastly, time...over time (it seems to vary between women) the
pain lessons.

Is mom under a lot of stress right now?  How is her marriage, her life with
3 other kidlings?  Often times we will find ourselves in more pain when we
are really stressed out - and when we relax, get a massage, get rid of the
stress (if possible) or deal with it, we do not have as much pain.  When I
am stressed out, my shoulders, back and right arm (from auto accident) ache
incessently (pain level of up to 8.5 on scale of 1-10) and I cannot function
well - I even feel MER while BF my 3/5 yr old! (something I do not normally
feel anymore)  When I relax and calm down, deal with what is stressing me,
the pain subsides and I can function better.
