first thing i would think of is edema. does she have edema this time and has 
she been shown and tried RPS? then i would think of maybe some endocrine 
abberations like thyroid. did she lose much blood? is she anemic? did she 
take a depo shot (I know you said she's on no meds)? any herbs? is she 
diabetic or gestational diabetes (large baby)? if so, are her sugars normal 
now? Of course there is retained placenta as a known connection to lack of 
lactogenesis II. Let us know how it goes.
