Rose asks about forms for use in insurance reimbursement in the USA.

There are insurance-related forms for IBCLC use that are sold by private
practitioners familiar with the byzantine rules regarding coding,
insurance, and privacy.  If you Google "lactation superbill IBCLC" you'll
pull up several credible options; a simple e-mail should confirm if they
are revised to reflect the newest options under the law.

It is an Insurance Jungle out there -- made more complicated by the
Affordable Care Act provisions that kicked in this year, and about which
even the insurance companies are still learning.  USLCA conducted a 3 hour
webinar in June 2013 with an insurance expert, about private insurance and
coding for lactation services in the USA.  It will be made available as a
recording.  I understand it was very informative; I'm planning to purchase
the recording myself.

Please keep your eyes peeled on the United States Breastfeeding Committee
website.  Later this month (July 2013)  there will be an evidence-based
model [insurance] payer guideline published that should make it *much*
easier for those of us who are helping mothers to navigate the insurance
maze to offer codes and language that will be recognized by private
insurers. (Bear in mind -- that will cover only 20% of the population.  30%
have "self-insurance" and 50% are Medicaid-eligible.  The laws regulations
and rules for insurance coverage under those systems is entirely
differently.)
