Here's another scenario for the discussion.  Had a mom who contracted to work
with me prenatally, attended my breastfeeding class, and she calls me from
the hospital needing the usual new mom reassurance on matters.  She ends up
going home with a pump from the hospital.  I go to her home for follow-up
yesterday, and she asks me the price of my rentals.  My rentals are $40 less
per month.  She now feels screwed, yet why didn't she feel she had the option
to deny the hospital rental and accept mine?  I realize I could have more
effectively counseled her on her options, but chose not to go there as I have
to be careful for conflict of interest issues since I work at the hospital
part-time.  So, I chose to say nothing....why should I have to be so worried
about any of this?  Why can't we more effectively align with each other to
partner on behalf of the patient?  I understand the difficulty of aligning
with poor quality/outcomes, but we're not talking quality issues here.  We're
talking about not providing patients with enough information so that they
make absolutely clear informed choices.  This patient told me if it weren't
for me and her comfort level with me since we'd established a relationship
prior to birth, she would have quit already.
