As for the SNS, she should try and tape the tube from the side. So if she is
using the cross cradle, tube should be taped at 3 o'clock on the right
breast, and 9 o'clock on the left. Also, in my old practice in NYC, my
colleagues and I found that using one tube as the feeding tube and the other
as the vent tube worked better, as opposed to taping both tubes to each
breast. Then you clamp both tubes, latch the baby, release the feeding tube,
and after 1 or 2 min. when the baby sucks rhythmically, you release the vent
tube. This might help with the clamping down on the tube only as well. 

As for the baby refusing the breast after using the Avent bottle, how about
using the SNS? If the medium tube is too slow, use the large one, and then
as baby gets used to nursing, slowly taper off the SNS.
