In the US hospitals are paid a set price for each delivery as negotiated between the insurance carrier (or state plan) and the hospital.  So even if the hospital is paying for the formula it uses, the parents will only be charged whatever is the copy/deductible  or percentage called for by their particular plan for their delivery and infant care.  You can request a itemized bill to review, I'm not sure formula would be listed as it is probably considered part of the infant care set.  

Our hospital does pay fair market value for the formula we use, since we are a BF hospital.  Many hospitals do receive it for free which of course is a corporate compliance violation..
