It sounds like you are right on with your plan of care. If at all possible, 
I think it would be best for baby to go to the 'well baby' nursery because 
(at least in USA) bf can be unrestricted. In the special nurseries the 
babies are usually placed on some schedule of feedings.
If possible have mom do as much kangaroo caring as possible. This helps with 
wt gain and thermoregulation.
I think it is a great idea for mom to express colostrum prenatally.
Your plan sounds very 'baby friendly'; make sure the mom and babe are 
followed up closely so if the baby is an ineffective feeder, you can protect 
the milk supply. I find these babies usually are hungry and feed well and 
gain fast.
