My hunch: poor latch at outset, due to gestational age and the effects of
augmentation on mother's possibly challenging nipples.

This spirals into poor intake and further compromised ability to nurse.  The
bili is *most* likely a factor of not eating enough.

I'll be surprised if simply increasing intake doesn't resolve this one
rapidly.  But I have been surprised before.
